At Clifton Family Church, photos and/or videos are taken during class time or class field trips or during special holiday events which the children take part in. These photos and videos may be uploaded to our Facebook page or on the Church’s Website, used in printed materials, or submitted with press releases to local publications, to illustrate our community for potential visitors. Children are never identified by name. It is the Church’s policy to respect the wishes of parents regarding photos or videos that include their children. Please tell us below if there are any ways you do not wish to use photos or videos including your children to be used publicly.

I, the undersigned parent or guardian, do hereby authorize emergency medical, dental, health or hospital services be rendered to my child upon the consent of the Unification Family Church Staff member or designated volunteer. The purpose of this authorization is to permit my child to receive emergency medical attention when needed while involved in the activities connected with the Unification Family Church programs when I or my emergency contact is unavailable to give such consent. Also as a Parent I will come and pick up my child from Sunday School if they are between the grades of PreK-2 Grade. This authorization shall be effective September 29, 2019 to June 28, 2020. *
